Office Policies

Fees/Billing:
The fee for the initial session (including psychosocial and diagnostic assessment) is $160.  The sessions are 50 minutes.  The fees for counseling services are as follows:

  • Intake Session (90791): $160
  • Individual Session
    • 38-52 minutes (90834): $110
    • 53+ minutes (90837): $130
  • Family/Couple: $130
  • Collateral (i.e.: court testimony, phone calls, etc.): $100/hour

Insurance Billing:
Clients will be responsible for your deductible and co-pays or co-insurance rates stipulated by your insurance company. 

We will bill your insurance company if we are on their panel for mental health and/or substance abuse counseling services. If we are not, the client may have the option of being billed as out of network or will be responsible for the full fee and may wish to submit claims directly to the insurance carrier.

Certain services are not always covered by insurance companies such as court ordered counseling. Please understand that if services are received under such a circumstance the client will be required to pay the full service fee at the time of service. It is the client’s responsibility to let the counselor know if such a circumstance exists. Any reports or court documents that need to be prepared by your counselor are done on a cash basis only. No insurance coverage is available for those services.

Payment:
Payment is expected at the time of service. Clients are responsible for co-insurance and/or co-payment amount.  If there is a deductible owing on the insurance policy the client will be expected to pay the full service fee at the time of the session until the deductible is paid in full.  If there is a balance owing on the bill a statement will be mailed. Failure to pay the bill in a timely manner as indicated on the bill may result in interest being added to the balance at the rate of 18% per annum (1.5% monthly). Returned checks will be charged a $25.00 fee plus any bank charges or fees.  Credit and debit cards are accepted.

Missed Appointments:
Missed appointments will be charged a fee set by your counselor if 24 hour advance notice is not given. Illness that prevents you from coming to your appointment will be taken into consideration. There is no insurance coverage for missed appointments.

Change of Client Information: 
Please notify your counselor if there are any changes to your information such as address, phone number and most importantly any changes in your insurance coverage.
